Referrals and Funding

A referral is not required to access any of the services at Involve Health Hub, however a referral may allow you to access rebated services through a range of schemes.

Medicare - Mental Health Care Plans

Mental Health Care Plan (MHCP) referrals allow for increased access to mental health treatment for individuals who are experiencing symptoms of mental health problems. Mental Health Care Plan referrals can be written by a General Practitioner, Paediatrician or Psychiatrist. A MHCP allows for up to 10 individual and 10 group rebated sessions per calendar year with eligible mental health professionals. All Involve Health Hub Psychologists and Accredited Mental Health Social Workers accept Mental Health Care Plan referrals.

Medicare - Eating Disorder Treatment Plans

Eating Disorder Treatment Plan (EDTP) referrals allow for increased access to both mental health and dietetic treatment for individuals who meet specific criteria for these referral types. A General Practitioner determines whether an individual meets the eligibility criteria for an EDTP, and if so, they can prepare the EDTP referral which allows for up to 40 rebated individual therapy sessions and up to 20 rebated dietetic sessions within 12 months from the date of the referral. Selected Involve Health Hub practitioners with experience and training in the area of eating disorders accept EDTP referrals.

Medicare – Chronic Disease Management Plans

Chronic Disease Management Plans referrals allow for increased access to multidisciplinary health treatment for individuals who experience chronic, terminal or complex health needs. Chronic Disease Management Plan referrals can be written by General Practitioners. A Chronic Disease Management Plan referral allows for up to 5 individual rebated health services each calendar year with eligible allied health professionals. All Involve Health Hub practitioners accept Chronic Disease Management Plan referrals.

Medicare - Complex Neurodevelopmental Disorder/Disability Referral (previously Helping Children with Autism Initiative pathway)

Complex Neurodevelopmental Disorder/Disability referrals allow for increased access to multidisciplinary health care for children and young people undergoing diagnosis, assessment or treatment for neurodevelopmental and other conditions. These referrals can be written by a Consultant Paediatricians, Psychiatrist, or General Practitioner (GP for specific diagnoses only). This referral allows for up to 8 rebated diagnostic or assessment services, and up to 20 rebated treatment services from eligible allied health professionals, throughout an individual’s lifetime. Involve Health Hub practitioners all accept Complex Neurodevelopmental Disorder/Disability referrals.

National Disability Insurance Scheme (NDIS)

The National Disability Insurance Scheme (NDIS) funds a range of supports and services, including multidisciplinary health care. Every individual who is approved for the NDIS has their own unique plan which outlines their needs, goals and required supports. This will determine what multidisciplinary health services you are eligible to access with NDIS funding. All Involve Health Hub practitioners are able to provide NDIS services to self and plan managed NDIS participants, provided your NDIS plan covers the relevant health service and it is relevant to your NDIS goals.

Private Health Insurance

If you are insured with a private health fund, you may be eligible for rebated multidisciplinary health services. In order to determine your eligibility for different health services, you will need to contact your fund and ask for the level of rebate to which you are entitled. Generally, private health funds require you to pay for your account in full, then reimburse you their rebate on presentation of your receipt from a health service.

WorkCover

WorkCover referrals fund multidisciplinary health services to assist workers stay, or return to work after an injury. Initial WorkCover referrals can be written by a General Practitioner who will assess your injury, health needs, and direct you to the relevant health discipline/s. The relevant health professional will then further assess your condition, then ongoing treatment must be approved by WorkCover. All Involve Health Hub practitioners accept WorkCover referrals.

CTP Insurance

If you are seeking health services related to a motor vehicle injury or incident, you may be eligible for CTP funded treatment. In order to determine your eligibility, you will need to contact your insurance case officer.

Employee Assistance Packages

Many workplaces offer limited health services through their Employee Assistance Programs. In order to determine your eligibility, you will need to speak with your workplace.

Department of Veterans Affairs (DVA)

The Department of Veterans Affairs (DVA) fund all health services necessary for Gold Card Holders, and some health services necessary for White Card Holders with particular health conditions. DVA referrals can be written by General Practitioners and allow for referral for up to 12 health services in one ‘cycle’. Clients can access as many referral ‘cycles’ as determined to be clinically necessary by their General Practitioner. All Involve Health Hub Psychologists and Accredited Mental Health Social Workers accept DVA referrals.

Victims of Crime

Each state of Australia has an independent Victims of Crime service which assists victims and witnesses of crime to access health services. To determine your eligibility for Victims of Crime funded health treatment, you will need to contact the service for the state in which the crime occurred, and make an application for assistance to access mental health care.
